Falling Oaks offers a discount for memberships paid with cash or check.  You may also pay via Square/Paypal.  Memberships are available to Medina County Residents.

• Complete the entire form
• Put the form and check in an envelope and put it in the Green Drop Box or mail it to 810 Guilford Blvd, Medina OH 44256

Credit Card

• Download the membership form above.
• Complete the entire form
• Put it in the Green Drop Box or Return it to 810 Guilford Blvd, Medina OH 44256
• Pay using our online marketplace
  • An additional fee is added to the price of items on our online marketplace to cover the service fee charged by the marketplace
